Municipality of Leidschendam-Voorburg: applying for youth care for your child
The municipality offers help with the safe and healthy growing up of all children up to the age of 18 and their parents. If you have general questions, you can contact the Social Service Point. For more complex questions, please contact a youth consultant. If you would like to apply for or extend youth assistance, please complete the application form.
What you need to know
- In 1 or more conversations, the Youth Consultant will examine with you what help is needed. You will receive a report of these conversations and, if necessary, youth assistance will be deployed
- You can contact us if one of the following situations applies to you:
You want guidance at home because of problems in the family
Your child has (serious) problems, such as behavioral problems or psychological problems
Your child has an intellectual disability
You think your child should be admitted to an institution
In all other situations where you are concerned - Do you like it if someone thinks along with you during the discussions with the municipality? Then you can use independent client support from MEE
- From the age of 18, guidance, daytime activities, overnight care, a protective living environment for people with a mental disorder or an aid or home adaptation fall under the Social Support Act (WMO)
- Since 2015, care for children and young people with severe mental, physical or multiple disabilities has been covered by the Long-Term Care Act (Wlz). These are children who are permanently dependent on permanent supervision or 24-hour care a day. You can request this care via the CIZ website
Above 18 years
- In some situations, help under the Youth Act is possible up to and including the age of 23. For information about the conditions, call the Youth Department on telephone number 14070
- Psychiatric or medical treatment falls under the Healthcare Insurance Act (Zvw) for anyone over the age of 18. Please contact your health insurer for this
- From the age of 18, guidance, daytime activities, overnight care, a protective living environment for people with a mental disorder or an aid or home adaptation fall under the Social Support Act (WMO)

You may draw up a plan yourself in which you indicate how you want to improve the situation. This is called the family group plan. You mention the following things in the plan.
