Municipality of Valkenswaard: applying for an Attestatie de vita (Certificate of life)

You can apply for an Attestatie de vita  (certificate of life) from the municipality where you live and are registered. You can only make the application yourself. You must make an appointment with the municipality to apply for a certificate of life.

You are registered as a resident of Valkenswaard or you live abroad and have Dutch nationality. A valid identification document. If you are requesting the Certificate of Life (Attestatie de vita) for a pension fund, also bring the letter from the pension fund requesting it.

A valid identification document, such as your passport or driver’s license.

A bank card. You pay directly at the time of the request.

A ‘Certificate of Life’ for European pension institutions is free upon presentation of a letter.

The ‘Certificate of Life’ (Attestatie de vita) must be requested in person at the town hall. You will receive the certificate immediately. Always make an appointment for this.
