Municipality of Olst-Wijhe: applying for an Attestatie de vita (Certificate of life)
You can only request proof of life from the municipality where you are registered.
Do you need proof of being alive abroad? Then the civil registrar must sign it.
For the application you need:
- a valid proof of identity;
- possibly the letter from your pension fund stating that you need a certificate de vita.
You must be present in person.
Arrange at the counter
Request your proof of life from the municipality. You must make online an appointment for this. An attestation de vita is an extract from the civil registry. You pay the costs for this extract upon application.
An attestation de vita for use abroad is an extract from the civil registry. An attestation de vita costs € 16.60.
Proof of being alive for use in the Netherlands is an extract from the Personal Records Database (BRP). A BRP extract costs € 12.25.
You pay this with the application.
The extract is free if your pension fund needs it. Your pension fund will ask you this in a letter. Show the municipality that letter when you request the extract.
You will receive the certificate immediately.
