You can apply for a certificate of life online. You need DigiD/Eidas to do this. You can also apply for a certificate of life at the service desk. First make an appointment. You can make an appointment online (in Dutch) or by phoning +31 71 36 60 000.

Bring to your appointment:

• your valid identity document;
• debit card (not credit card) or cash.

Attestation de vita

You can apply for an Attestation de vita at the service desk. First make an appointment. You can make an appointment online (in Dutch) or by phoning +31 71 36 60 000.

Bring to your appointment:

• your valid identity document;
• any document that needs to be legalized;
• debit card (not credit card) or cash.

A certificate of life costs € 5,75. And an Attestation de vita costs € 16,60.
