Applying for an Attestatie de vita (Certificate of life) in the Netherlands
The Attestatie de vita is a document that you can use outside the Netherlands to prove that you are alive. Some organizations, such as a pension fund, may request you to produce this document. In the Netherlands you can use the document Bewijs van in leven zijn (Proof of being alive) for the same purpose.

You can apply for a certificate of life online. You need DigiD to do this. You can also apply for a certificate of life at the service desk. First make an appointment. You can make an appointment online (in Dutch) or by phoning +31 71 36 60 000.
Bring to your appointment:
- your valid identity document;
- debit card (not credit card) or cash.
Attestation de vita
You can apply for an Attestation de vita at the service desk. First make an appointment. You can make an appointment online (in Dutch) or by phoning +31 71 36 60 000.
Bring to your appointment:
- your valid identity document;
- any document that needs to be legalized;
- debit card (not credit card) or cash.
A certificate of life costs € 5,75. And an Attestation de vita costs € 16,60.
