Municipality of Maastricht: applying for an Attestatie de vita (Certificate of life)

You need to request an Attestatie de vita (certificate of life) from the municipality where you live and are registered. Only you can make this application; it can't be done on your behalf. You can apply for the Attestatie de vita online, or you can book an appointment at the GemeenteLoket (municipal service desk).

For us to process your application, we'll need the documents listed below. You can upload these during the online application.

If you're visiting the GemeenteLoket (municipal service desk) for your Attestatie de vita, bring these documents to your appointment:

  1. the completed application form or your pension fund's letter bearing your signature 
  2. a copy of your ID (driver's license, passport or identity card)

You can only apply for an Attestatie de vita from us if you reside in Maastricht. If you live in a different municipality, please apply there.

If you need an Attestatie de vita for your pension fund, it's free of charge. Simply attach the request letter from your pension fund to your application.
