Municipality of Hilvarenbeek: applying for an Attestatie de vita (Certificate of life)
You can apply for an Attestatie de vita (certificate of life) from the municipality where you live and are registered. You can only make the application yourself. Someone else cannot do it for you. You must make an appointment with the municipality to apply for a certificate of life.

If you request the extract at the counter of the town hall, please bring:
- A valid ID
- Do you have a letter from the pension fund? Take this with you. Then the application is free
- Are you requesting the extract for someone else? Please also bring the completed and signed authorization form with you. And a (copy) proof of identity of the applicant
You pay a fee for this service. For more information, contact the municipality.
You will receive the certificate immediately.
