You can apply for an Attestatie de vita (certificate of life) from the municipality where you live and are registered. You can only make the application yourself. Someone else cannot do it for you. You must make an appointment with the municipality to apply for a certificate of life.

- You are registered as a resident of Geertruidenberg or you live abroad and you have Dutch nationality.
- You can apply for a certificate of vita for yourself or for your child under the age of 18 who lives with you. The child must be present during the application.
- An application for an attestatie de vita must be submitted in person at the counter. You will then receive the proof immediately.
- An attestatie de vita is an international document in Dutch and French.
- A valid identity document, such as your passport or driving licence.
- Cash or a payment card. You pay the fee at the time of application.
A certificate of life costs: € 16.20
You pay a fee for this service.
You will receive the certificate immediately.
