You can apply for an Attestatie de vita (certificate of life) from the municipality where you live and are registered. You can only make the application yourself. Someone else cannot do it for you. You must make an appointment with the municipality to apply for a certificate of life.

- Cash or a payment card. You pay the fee at the time of application
- If necessary a letter from the pension fund
- If necessary an authorization form schriftelijke machtiging
Who can you authorize?
- Persons living together
- Parents, caretakers or guardians and other family members (16 years or older)
- Others (non-family members of the applicant), regardless of the applicant's age
- Persons placed under guardianship (authorisation of trustees)
A certificate of life for the Netherlands costs: € 13,-
A certificate of life for abroad costs: € 16,60
You will receive the certificate immediately.
