You can apply for an Attestatie de vita (Certificate of Life) at the municipality where you live and are registered. You can only submit the application yourself. You must make an appointment with the municipality to apply for a certificate of life, or apply for the certificate online.

If you make an appointment you will receive the certificate immediately.
If you apply digitally the certificate will be sent to you by post.
