Municipality of Arnhem: applying for an Attestatie de vita (Certificate of life)
A Certificate of life or Attestation de vita is written proof that you are alive. Benefit agencies such as (foreign) pension funds may ask for this. To apply, make an appointment.
Bring the following to the appointment:
- Valid proof of ID.
- Does your pension fund ask for the Proof of Life or Attestation de vita? Then bring the letter from your (foreign) pension fund with you to the appointment.
Authorize someone
- To apply for a Certificate of Survivorship, you can authorize someone.;This person can then apply for the certificate on your behalf.
- To apply for an Attestation de vita, you must come in person. You cannot authorize someone.
To apply for a life certificate or a declaration of existence you have to make an online appointment.
A life certificate costs € 12,20, unless you present a letter from your pension fund. If this shows that you must be able to provide the life certificate, there is no charge.
A declaration of existence costs € 17,10, unless you present a letter from your pension fund. If this shows that you must be able to submit the declaration of existence, there is no charge.
You will receive the certificate immediately.
