Municipality of Apeldoorn: applying for an Attestatie de vita (Certificate of life)

A Proof of Being Alive shows that you are alive at the time the certificate is issued. A proof of being alive is intended for use by Dutch authorities. For foreign authorities, you will need an Attestatie de vita.

You are registered in the municipality of Apeldoorn.

A Proof of living is intended for use by Dutch authorities. For foreign authorities, you will need an Attestatie de vita.

You can apply for a Certificate of living will in two ways:

Online

Apply for the proof online

The certificate will be sent to your home address by post within three working days.

Counter

Make an appointment online to apply for the certificate.

Bring with you to the appointment:

A valid proof of identity
The letter from your pension fund if you request the certificate for it

You will receive the certificate immediately.

Authorisation

You can authorise another person to apply for and collect a proof of living for you. Please use the authorisation form (pdf, 155 kB). The authorisation may not be a scan or copy. The application requires your original signature. So you cannot e-mail the form to the person coming to make the application on your behalf.

If your pension fund asks for the proof, attach it to your application, the application will be free of charge.

If your pension insurance (life insurance) asks for the proof, the cost is for yourself.
