Municipality of Amstelveen: applying for an Attestatie de vita (Certificate of life)

You can apply for an Attestatie de vita (Certificate of life) from the municipality where you live and are registered. You can only make the application yourself. You must make an appointment with the municipality to apply for a Certificate of life. 

Do you need to provide a Certificate of life to a Dutch authority? You can apply for this document online. You will need your DigiD for this. You can also apply for the certificate at the service desk in the town hall. You need to make an appointment in advance for this via the municipal website.

You will need the following for an online application:

  • DigiD.

You will need the following for your appointment at the town hall:

  • A valid ID, such as your passport or ID card. 
  • A notification from the relevant authority stating that you need the proof (not mandatory). 
  • Cash or a bank card. You will need to pay instantly upon application.

You need to pay for the application of an Attestatie de vita or a Certificate of life. For more information, please see the municipality website. If your pension fund is asking for proof that you are alive, then the proof is free of charge.

  • For applications made online, the municipality will send the document to you by post within 5 working days. 
  • For applications made at the town hall, you will receive the document immediately.
