Municipality of Valkenswaard: registering a stillborn baby

You submit a request to the municipality where you live.

To register your child, a certificate of birth (stillborn) or a certificate of stillborn child is required. If you do not have such a certificate, you can have one drawn up in the municipality where your child was born.

It may happen that the names of your child are not on the certificate of birth (stillborn) or certificate of stillborn child. In that case, you provide the names when you request registration in the BRP (Personal Records Database). Parents can each provide a different name separately. This is not a problem in this situation.

What do you need for a written request?

  • The completed form for registering the child on the personal list: form for registering the child on the personal list
  • A (copy of a) valid ID
  • A certificate of your stillborn child or a certificate of birth (stillborn) that is recorded in the death register of the Dutch civil registry

What to bring to your appointment?

  • A valid ID
  • A certificate of your stillborn child or a certificate of birth (stillborn) that is recorded in the death register of the Dutch civil registry

A certificate of birth (stillborn) can be drawn up in the municipality where your child was born.
