Municipality of Westervoort: registering a death

• Your proof of identity (passport, identity card or driving licence).
• A doctor's certificate of death.
• B-envelope from the Central Bureau of Statistics (CBS) containing details of the cause of death. You will get this from the doctor.
• If necessary, the deceased's marriage booklet.

Declaration

Within 6 days of death, the death must be registered at the registry.
