Declaration by surviving dependants
Please bring the following documents with you to the appointment:
- B envelope containing the doctor's form about the cause of death
- death certificate (from a medical examiner)
- possibly the marriage certificate
- valid proof of identity of the person making the declaration
The term within which the cremation or burial must take place is no later than the sixth working day after the death. If an extension of this term is necessary, please also bring:
- statement from a doctor showing that there is no objection to postponement of the term of burial or cremation.
In case of an unnatural cause of death is also required:
- certificate of unnatural death (this will be sent directly to the municipality by the coroner)
- declaration of no objection for the burial or cremation of the public prosecutor (this is sent directly to the municipality from the judiciary).
Declaration by surviving dependants
You can register the death yourself. You must do this in the municipality where your loved one died. Make an appointment with the municipality of Moerdijk
Online registration by funeral directors
You can also ask the funeral home to register the death for you. Funeral homes who are located in the Netherlands can register deaths online.
- Declaration of death: free
- Copy of the death certificate: € 15,70.
- A multilingual extract from a civil status certificate costs € 15,70.
- Burial or cremation leave: free
- Leave buried earlier or later: free
- Leave for transport abroad (laissez-passer): free
- As a surviving dependant, you receive the extract from the death certificate and the verlof tot begraven of cremeren (permission for burial or cremation) immediately.
- Funeral homes should receive the extract and any other documents on the same working day or, at the latest, the next working day.
