Municipality of Apeldoorn: registering a death

If someone has died, you report this in the municipality where the person died. The municipality registers the death of this person in the civil registry and the Basic Registration of Persons (BRP).

You can only file a declaration if:

  • The person died in Apeldoorn.
  • The burial or cremation takes place within 6 working days of the day of death.

Declaration by surviving dependants

Make an appointment online to declare the death.

Bring with you to the appointment:

  • A valid proof of identity of yourself
  • A doctor's certificate (A-declaration) of death
  • The B envelope (envelope for the Central Bureau of Statistics containing a medical statement of the cause of death)
     

We will process your declaration immediately.

You can report the death in two ways:

Extract from the death certificate
The payment (iDEAL)
Uploading the online declaration:

The A-declaration
Living will declaration (if applicable)
Permission to defer from the doctor (if applicable)
You must bring or send the B envelope and will statement by the first of each month.

We will process your declaration within 24 hours on working days. You will receive the requested documents via myapeldoorn.nl (under Burgerzaken).

The declaration of death is free of charge. Do you also want a copy of the death certificate? There is a charge for that.
