Municipality of Zwolle: registering a birth

Needed for online declaration:

• DigiD login details of (both) parent(s). Parent(s) must be registered in the BRP.
• If your child was recognized before birth or if you chose a name before birth, it is mandatory to upload the deed of acknowledgment or deed of choice of name.

Required for birth registration at the city office:

• Valid ID
• Certificate of birth from the midwife or hospital.

If applicable, also bring the following:

• marriage booklet or partnership booklet;
• anonymous donor statement issued by the Foundation for Donor Data Artificial Fertilization via donorgegevens.nl
• act of recognition 
• deed of choice of name.

Inform the municipality directly of the birth of your child. You can do so:

• Online. doorgeven met DigiD
• At the service desk. You can book an appointment for this.

Registering a birth is free of charge. A birth certificate costs: € 17,10.
