Registration
You can register a birth online or make an appointment with the municipality.
Choice of surname
The child can be given the surname of the mother or father. All children that you subsequently have together will automatically have the same last name. The procedure depends on your situation:
Are you married?
Then your child will automatically be given the father's last name. Do you want your child to have the mother's last name? In that case, you must have the municipality draw up a certificate of name choice for the birth registration. You can make an appointment for this with the municipality. Please take along the mother to your appointment.
Are you not married?
Then your child will automatically be given the mother's surname. Do you want your child to
have the father's surname? Then the father must acknowledge your child. The new choice of surname will appear on the official deed.
Are you getting married later?
Then your child can still be given the father's surname after getting married. You can make an appointment for this with the municipality. Please take along the mother to your appointment.
Was your child born abroad?
Then you can change your child's surname within 2 years after birth.

If you register the birth of a child online:
-Your DigiD.
-A birth certificate or birth report from your doctor or midwife.
-A certificate of acknowledgement of the child (if this is applicable).
If you register the birth of a child at town hall:
-A valid ID.
-A birth certificate or birth report from your doctor or midwife.
-A certificate of acknowledgement of the child (if this is applicable).
--Your marriage booklet or partnership booklet (if you want your child to be mentioned in this).
A statement of parentage in case of dual motherhood (if the donor is unknown).
Registering a birth is free of charge.
- Immediately upon birth, your child will receive a social security number (BSN).
- Your child will be registered in the municipality in which the mother is registered.
- After a few days, the BSN is also listed on Mijn Overheid (under 'Identiteit' and 'Familie') and you will receive the BSN by written mail. With this BSN you can take out health insurance for your child.
