Municipality of Hoeksche Waard: registering a birth

Pass it on online:

• Your DigiD.
• Statement from the doctor or midwife. Please note: you must add this statement digitally to the online declaration.
• Deed of name selection (if you have one). Please note: you must add this statement digitally to the online declaration.
• Unborn child recognition certificate (if you have one). Please note: you must add this statement digitally to the online declaration.

When you come by:

• Valid proof of identity of the person making the declaration.
• Valid proof of identity of the mother.
• Deed of name selection (if you have one).
• Unborn child recognition certificate (if you have one).
• Statement from the doctor or midwife (if you have one).

Not mandatory; handy:

• Wedding booklet (only if you want the child to be included).
