Municipality of Dijk en Waard: registering a birth
Was your child born in Dijk en Waard? Then you can declare the birth in 2 ways. You file an online declaration with DigiD. Or you register the birth in person at the town hall, for this you first make an appointment.

- At the service desk Contact us to make an appointment.
Before you start the registration process:
- Choose your child's first name(s). Pay close attention to the spelling, as it cannot be changed later.
- Have the date and time of birth ready.
- Have the midwife's name ready.
- Have the birth certificate/statement provided by the midwife or doctor ready.
- Have the akte van erkenning (deed of acknowledgement) ready, if you are not married and choose the father's surname.
- The mother must log in using DigiD. If you wish to upload a deed of acknowledgement, the father or co-mother must also log in using DigiD.
Registering a birth is free of charge.
There is a fee for a birth certificate. For more information, contact the municipality.
