Municipality of Amstelveen: registering a birth
You need to report the birth of a child to the municipality within 3 days. You can do this at the counter in the Town Hall.
Make an appointment online for your visit to the Town Hall (in Dutch)

- A valid passport, identity card or driving licence.
- A valid passport, identity card or driving licence of the child’s mother.
If applicable, also bring one of the following documents:
- Declaration of birth from the hospital or midwife (if you have one).
- Your marriage or partnership booklet, if you want the municipality to include your child’s name in it.
- A copy of the certificate of acknowledgement of parentage if the child was acknowledged before birth.
- A declaration of last name choice, only for the birth of your first child.
- A statement from the Artificial Insemination Donor Data Foundation (website in Dutch), if applicable.
- Your (legalised) marriage certificate if you were married abroad and it has not been registered in the Personal Records Database (Basisregistratie Personen; BRP).
The registration of a birth is free. You do pay costs for a copy or extract of the birth certificate or for a birth booklet. For more information, please see the municipality’s website.
