Municipality of Heemskerk: applying for a Verklaring Omtrent het Gedrag (VOG)

• Your valid proof of identity (passport, driver's license, Dutch identity card or residence permit).
• A fully completed and signed application form. You will receive the application form from the person who needs a VOG from you. This may be an employer, an association or a government agency. The form states the purpose of the application.

Print the application form from the Justis, the Ministry of Justice and Security’s screening authority and fill it in together with your employer. Hand in the form at the municipality where you are registered. You must also take proof of identity with you, such as your passport or driving licence.

You pay a fee for this service. The cost of a VOG can be found on the municipality's website.
