Municipality of Ommen: applying for an EU parking card for people with disabilities
Please note: You can only apply for an EU parking card for people with disabilities if you live in the Netherlands and are registered with a municipality.
For the application you need:
- Copy of a valid passport or identity card.
- If you are applying for the driver card: copy of a valid driving licence.
- A colour passport photo that is of a good likeness to you and is not older than 6 months.
- If you have a Canta microcar or motorised tricycle/quadricycle: a copy of the insurance certificate.
Medical advice
The municipality can request medical advice from an independent doctor. That is never your own GP. The municipality of Ommen employs Argonaut for this.
Based on the medical advice, it will be determined whether you can be provided with a GPK. Medical advice is always requested when you apply for a GPK for the first time.
Complete the form and include the attachments.
