Municipality of Noordwijk: applying for an EU parking card for people with disabilities

You can only apply for an parking card for people with disabilities if you live in the Netherlands and are registered with a municipality. You can apply at the ISD Bollenstreek by downloading a form (in Dutch). View for contact details the Dutch-language website isdbollenstreek.nl.

Complete the form and include the attachments:

• 1 recent passport photo;
• a copy of your valid identity document;
• a copy of your driving licence by applying a driver’s card;
• your old parking card for people with disabilities by renewing your parking card for people with disabilities.

Send the completed form with the include attachments to the ISD Bollenstreek, Postbus 255, 2160 AG Lisse, the Netherlands.
