Municipality of Amsterdam: applying for an EU parking card for people with disabilities

For the application you need:

• Copy of a valid passport or identity card.
• If you are applying for the driver card: copy of a valid driving licence.
• A colour passport photo that is of a good likeness to you and is not older than 6 months.
• If you have a Canta microcar or motorised tricycle/quadricycle: a copy of the insurance certificate.

How to apply

• Online: use the form Disabled parking card application (in Dutch).
• By mail: download the application form and send the form and documents to the City Office of the district where you live. The addresses are on the form.

• EU parking card for people with disabilities: €44
• Medical assessment: €  165.70

It will take 8  weeks to get your card. You must collect the card in person at the City Office.
