Municipality of Oisterwijk: applying for a copy of a birth certificate

• When applying for yourself and/or family members living at home: only your own proof of identity
• Money: you pay the extract or statement with the application in cash or with debit card

If you have been authorized by someone to request an extract or copy, you must bring:

• The completed authorization form
• Your own proof of identity
• A valid proof of identity of the person who authorized you or a copy thereof
• Money: you pay the extract or statement with the application in cash or with debit card

You can request an extract or copy in 3 ways:

• online: You log in with DigiD. You pay with iDeal and the extract or statement will be sent to your home within 3 working days. If you have an authorization, you cannot apply online.
• by appointment: at the public desk in the town hall in 
• writing: send an e-mail to gemeente@oisterwijk.nl. You will receive the bill separately
