Municipality of Midden-Delfland: applying for a copy of a birth certificate
The registry office is a department of the municipality. The registry office creates and stores official documents of important events. These documents are called deeds.
Examples of deeds:
birth certificate
marriage certificate
divorce certificate
death certificate
You can request a copy or extract of a deed from the municipality where the deed was drawn up.
Copy or extract A copy is a copy of the deed with a stamp and signature of the civil registrar. An extract contains only the personal data.
What should I do?
You can request a copy or extract from the civil status records at the municipality where the deed was drawn up. Indicate why you need the copy or extract.
Request an extract
You can request an extract for the following persons:
- For yourself.
For your child who lives with you. Even if he or she is older than 16 years.
For someone else, if this person authorizes you to do so. This can be done in a letter in which he or she writes that you are given permission to act as an authorized representative. International extract.
An extract from civil status is available in several languages. Please indicate with your application that you want an international extract. A copy of the civil status is only available in Dutch. If you need a copy in another language, you must have the copy translated by a sworn translator yourself.
Empower someone else
If someone else submits the application for you to the municipality, you must give them a letter containing:
- the reason why you need the copy or extract;
the name of the authorized representative;
your signature;
the signature of the authorized representative.

What do I need?
Apply online
You request the statement online using your DigiD login code. Once your payment has been received, the extract will be sent to you by post.

your valid proof of identity (passport, driver's license, Dutch identity card or foreigner's document)
debit card (or cash) to pay.
Request in writing
- copy of your valid ID
letter stating why you need the copy or extract
your signature on the letter

Use the Authorization Form under the heading 'Forms'. You can also create your own letter with the following information:

to whom you give permission to collect the copy or extract (the authorized representative)
your signature on the letter
the signature of the authorized representative
- The authorized representative takes this letter to the Civil Affairs desk together with:
a copy of your valid ID
his or her own ID
