Applying online

Were you born in the Netherlands? Do you live in the Netherlands and have a DigiD? If so, you can apply for your birth certificate online. You can also use your DigiD to apply for your birth certificate from outside the Netherlands.

You first transfer € 14.30 to Publiekszaken DVL/PRS, IBAN number: NL25 INGB 0001 0960 58 (or BIC code INGB NL2A). State your name, the application date and 'excerpt civil status' in the description. Then fill in the form. You enclose proof of payment and a scan of your proof of identity.

Applying at the service desk

You can apply for your birth certificate at the service desk of your municipality:

Stadhuisplein 1, 5611 EM in Eindhoven. Take a valid identity document with you.

By appointment

Call us on +31 40 238 6000 before making an appointment to come to the City Hall.

Without an appointment

On Tuesdays and Thursdays between 8:30 and 11:00 am you can walk in for this product without an appointment (for events after 1970).

Applying in the Netherlands

You will receive the birth certificate by post after no more than 6 working days.

Applying from outside the Netherlands

The municipality will send the birth certificate to your address outside the Netherlands within 5 working days. The time taken by the postal service varies from country to country.
