Municipality of Noordwijk: acknowledging parenthood of your child

Acknowledging parenthood of your child is possible in every municipality in the Netherlands. At the municipality of Noordwijk you first make an appointment. You can make an appointment online (in Dutch) or by phoning +31 71 36 60 000.

You will need the following for the appointment:

• When acknowledging parenthood of the first child, the father and mother must be present at the appointment. You choose whose surname the child will have. Choice of surname is possible before the birth or when registering the birth. All subsequent children from the same relationship are automatically given the same surname as the first child. Bring a valid identity document of the father and mother.
• Will the mother not be present at a subsequent child? If so, she must give her written consent.
• Are you acknowledging parenthood of a child during pregnancy? Please keep the copy you receive after acknowledging parenthood of your child. You need the copy when registering the birth.
• Is your child over the age of 12? If so, you will need consent from the child. The child must be present at the appointment or you can bring a written consent from the child.

Acknowledging parenthood of your child and choose a surname are free of charge.
