Municipality of Midden-Groningen: acknowledging parenthood of your child

• A valid ID.
• Do you want the first child to be given the father's name? Then the mother has to come along to the town hall. She must bring a valid ID.
• Is the mother not coming to the appointment? Then you need the written consent with the mother's signature. You should also bring a copy of the mother's valid proof of identity. This is possible if a next child is born from the same relationship.

Official acknowledgement and choice of surname are free of charge. You pay a fee for an akte van erkenning (acknowledgement of parenthood certificate). For more information, contact the municipality.
