Municipality of Koggenland: acknowledging parenthood of your child
- You wish to become the legal parent of your (unborn) child.
- At the time of the child's birth, you are not married to the mother.
- At the time of the child's birth, you do not have a registered partnership with the mother.
- If both the father and the mother only have Dutch nationality, you can make an appointment.
Co-mother
- Please note: If you are the co-mother, you must bring the "unknown donor" declaration when registering the birth. If you do not do so, you can still formally recognize your child and become the child's legal parent. For more information, see the information about co-motherhood and legal parenthood.
If the mother accompanies you to the town hall, bring:
- Your valid identity document.
- The mother's valid identity document.
If the mother does not accompany you to the town hall, bring:
- Your valid identity document.
- Written consent from the mother, including her signature.
- The mother's valid identity document.
If you or the mother live abroad, you must also provide an official document from the relevant foreign authority confirming that you are not married.

Official acknowledgement and choice of surname are free of charge. You only pay for a certified copy of the deed of recognition: €17,80.
