Municipality of Apeldoorn: acknowledging parenthood of your child

Are you not married or in a registered partnership with the child's mother? Or are you a duo mother? Then you can acknowledge your child. You do not have to be the biological parent. You can acknowledge a child before the birth, when the birth is declared or after the birth. The options have different consequences.

Bring with you to the appointment:

A valid proof of identity of yourself
Acknowledgement first child:

The mother must come with you if you choose the surname of father/duo mother. She too brings a valid proof of identity.
Written consent (pdf, 106 kB) together with a copy of a valid proof of identity of the mother if the child is given the surname of the mother.
Acknowledgement of second or subsequent child

Written consent (pdf, 106 kB) together with a copy of a valid proof of identity of mother.
